Fees include all conference materials, breakfast and lunch daily, welcome reception & entertainment.
Policies Payment will be accepted in Canadian funds by cheque or money order. All cancellations or transfer requests must be in writing and will be subject to an administration fee of $150. Any cancellations received on or after the opening day of the conference will be subject to all registration fees. The conference registration form will be completed submitted online directly to CHCAA conference committee. To complete your conference registration, please mail the applicable registration fee to: Ms. Jacinta Khan
Payment of the registration fee is required before confirmation of conference attendance will be sent. Hotel Accommodation The Courtyard by Marriott - Downtown Toronto is the 'hotel of choice' for the CHCAA 2nd Annual Conference. A number of rooms have been reserved from September 25-27, 2001 at a special group rate ($129 + taxes for a single or double). Please make room reservations directly with the Courtyard Marriott 1-800-847-5075. To ensure you receive the special group rate, please use the group name CHCAA when making your reservations. All reservations must be guaranteed with a credit card prior to arrival. All reservations must be received by September 3, 2001 to receive the special group rate and to guarantee an available room! |
